If you are unable to obtain a Criminal Record Check with Vulnerable Sector Screening due to age restrictions, you must complete this form and submit it with your clinical document package to Synergy. If you turn 18 during the academic year, you are required to submit a Criminal Record Check with Vulnerable Sector Screening at that time.
