Discover + Adopt (D+A) Program

Eligibility: 

Applicant Eligibility:

  • The applicant must be an organization that provides care to older adults (e.g., long-term care home, retirement home, hospital, home care and/or community-based organization).
  • The applicant must be located in Canada.
  • If the applicant is a research or academic institution, they must be affiliated with a healthcare delivery organization that provides care to older adults, where the solution will be tested and implemented.
  • The applicant must have staff members who can participate in all program activities (e.g., learning series, coaching update, planning, etc.)
  • The applicant must have leadership support to participate.
  • Applications must identify a project lead who wil be responsible for overseeing the project and provide regular updates to CABHI.
  • Project leads must be employed at the organization and are typicaly individuals in the organization who lead programs and initiatives, including but not limited to, executive leaders, directors of care, clinical managers, operations managers, program mangers, quality improvement specialists, innovation leads etc.
  • Applicants who have participated in three (3) cohorts are no longer eligible to apply.*
  • Applicants who are currently participating in a cohort are not eligible to apply.*

Calling all Canadian healthcare and community-based organizations! Bring innovation into your care setting with the Centre for Aging + Brain Health Innovation (CABHI), powered by Baycrest, and its Discover + Adopt (D+A) program.

The D+A program, now accepting applications, offers up to $150,000 CAD in funding and tailored support services to help Canadian healthcare and community-based organizations identify, test, and implement solutions that improve quality of care and quality of life for older persons and their care teams.
