  • Departmental PTA Form (for appointments initiated within an academic department and must be approved by the Department Chair before submission to the Dean’s office)
  • Non-departmental PTA Form (for appointments outside academic departments, including those reporting directly to the Dean, and is submitted directly to the Dean’s office for approval)
