Consent of Disclosure Form

Lakehead University is generally bound by law to keep information about you in its records in strict confidence.  Complete and submit the Consent of Disclosure Form if you wish to allow the University to disclose this information to others.

Admission Profile - Bachelor of Education Applicants

Applicants to the Bachelor of Education Degree Program may be required to submit the Admission Profile Form to supplement their application. Please only complete and submit this form at the request of an Admissions Officer. 

Special Consideration - General Admissions Form

Applicants to an undergraduate program may be required to submit the Special Consideration Form to supplement their application.  Please only complete and submit this form at the request of an Admissions Officer. 

Re-admission Profile - General Admissions Form

Previous Lakehead University students may be required to submit the Re-admission Profile Form to supplement their application. Please only complete and submit this form at the request of an Admissions Officer. 

Nursing Consent - Program Specific Form

Applicants to the Bachelor of Science in Nursing program who have attended a Nursing program at another university will be required to submit a signed Nursing Consent Form. Please complete and submit this form at the request of an Admissions Officer.

Bachelor of Education Placement - Program Specific Form

Placement forms are required for applicants who receive an offer of admission into Lakehead University’s Bachelor of Education Program.

Bachelor of Education Intermediate-Senior Teaching Subject Requirement Form

This form is required for all applicants to our Intermediate-Senior programs. 

Please complete and submit the Teaching Subject Requirements Submission Form through your student portal under the Required Documents section.
