Nursing Records Check Letter for BScN Students

The form below is to be used when applying for a Criminal Record Check with Vulnerable Sector Screening. It is a fillable PDF form, and you must enter your name and the date after downloading it. If these fields are left blank, the form may be rejected.

 If you are under the age of 18, please complete the Interim Criminal Offence Declaration form.

You must apply to the Police Agency where you currently live. If you are living out of town, you cannot apply with Thunder Bay Police. Once you are living in Thunder Bay attending school you can, at that point, apply for your CRC VSS with Thunder Bay Police. The Thunder Bay police refer to the CRC VSS as "Level 3:Police Vulnerable Sector Check (PVSC)".

If you are completing your CRC through the Toronto Police Service, you must apply online. Visit www.tps.to/police-checks for more information and to access the application. You will need to apply using the Lakehead University School of Nursing agency code, provided below.
