Documentation can be provided to Student Accessibility Services

or in person to the Thunder Bay office, SC 0003.

Students can provide scans or pictures of documentation in lieu of the physical document. If possible, providing documentation in advance of meeting with an advisor is appreciated. 

Documentation Guidelines - Guidelines for who can provide documentation of a disability and/or medical condition and what information must be provided on the documentation.

Medical Documentation Form - A fillable PDF form that can be printed or emailed to your care provider to ensure we have all the information we need. The form can then be dropped of in person, faxed, or emailed to sas@lakeheadu.ca.

SAS Service Animals on Campus Documentation Form - A fillable form that accompanies medical documentation for students who are supported by a trained service animal in their classes and exams.
