THIS FORM IS FOR APRIL TIMETABLED EXAMS ONLY (April 10th - 19th) . DO NOT BOOK MIDTERM TESTS or REGULARLY SCHEDULED ONLINE EXAMS (that are not scheduled to be written in a classroom on campus) ON THIS FORM.

 

Please make sure you are logged in to your Lakehead email account.

You must submit a separate form for EACH exam booking request.

For Orillia exams please check in for your Exam at OA 1017 (Test Centre) with your student ID 10 minutes prior to the start of the exam. You will ONLY be notified by email if SAS cannot complete your request.
