- Complete this form if you are a Faculty member who wishes to apply for membership to the Faculty of Graduate Studies. Applications are to be submitted to your Graduate Coordinator. View the nomination process for Faculty designation.
- Curriculum Vitae
- In addition to the Membership Nomination Form, you must submit a paper and electronic copy of your curriculum vitae to the Faculty Dean who oversees the program to which you are applying.
- Your curriculum vitae must be in the IQAP format.
